A ‘case’ can be a crime, a situation with a student in school or nearly anything that comes up on a consistent basis. In this idiom, it means that each time it comes up, it can be dealt with according to specific aspects of the situation.

When disciplining students, each situation must be dealt with on a case by case basis.

When there is a traffic accident, the police investigate and deal with it case by case.

When a problem comes up at work, the boss deals with it case by case.

When someone misunderstands, the teacher must deal with each student case by case.
