I completely apologise. (*More formal in written than in speaking)

I am ever so sorry (more weight and geniune)
我实在很抱歉
I messed up 
我搞砸了
I was (completely) in the wrong
我（完全）错了
I was (completely) in the wrong when I cut off you call yesterday.

I know I hurt your feelings
e.g I know I hurt your feelingg when I refuse to attend your brother's wedding!

Reponse to apologies: 
No worries/That's alright!
More British: No harm done/NO hard feelings (I don't hate you, we are good)
Well, you couldn't help it!
It's water under the bridge = we had disagreement, now those bad feelings are gone.
It doesn't matter
